Purpose. You are invited to participate in our study, Eating and Health Risk Behavior. The purpose of this research is to study health risk and eating behavior in women and men. We are interested in learning about you as a person, your background, your health risk and eating behaviors, and their effect on your personal wellness.
Your participation. We welcome anyone 18 years or older to take part who is English speaking. We encourage a diversity of participants from around the world willing to take our short, 15-minute survey.
Your rights. Your decision to participate in this study is entirely voluntary. This means you may decide at any time to withdraw from the study. If you do not wish to submit your responses, simply click the browser page closed. Should you wish to withdraw your responses after submitting them, please e-mail Dr. W. Andrew Harrell with the approximate time and date that you completed the questionnaire, and he will remove your responses from the study.
Anonymity and Confidentiality. Your responses will be anonymous, and they will remain confidential. Your name will not appear on the questionnaire or be associated with your responses in any way. Only researchers associated with the project will have access to the questionnaires. You may obtain a summary of results by e-mailing Dr. Harrell. The results of this study may be presented at scientific conferences or published in journals. No individual results will be released; only grouped (aggregated) data will be presented. The data will be safely stored by Dr. Harrell for a minimum of seven years.
Benefits and risks. A personal benefit of completing the questionnaire is that it may stimulate your thinking about your eating habits, attitudes and behaviors. This research will contribute to the advancement of our understanding of eating behavior, including body image and eating concerns in women and men, along with factors that might affect this behavior.
There is no known risk to you as a participant. The questionnaire does address some sensitive issues, and you may find some of the questions unsettling or upsetting. You do not have to answer any questions that make you feel unsettled or upset.
